Thank you for booking your appointment at The Norfolk Mead Treatment Rooms. Please read the following information carefully about what you need to know before attending your appointment. All of the following measures have been put in place for the safety of you and our staff.
Please complete our consultation forms before you attend your appointment by clicking on the links:

If you can not do this online then please email us via relax@norfolkmead.co.uk and request the form to print out at home, complete and bring to your appointment.

Only arrive a maximum of 5-10 minutes before your appointment time.
Arrive through the main hotel front door and wait at the main hotel reception to be greeted by our front of house team. If a member of our team is not at the hotel reception desk when you arrive please wait and someone will be with you as soon as possible.
If you have been around anyone who has displayed COVID-19 symptoms or if you have developed symptoms yourself, then please let us know as soon as possible. Our usual cancellation terms and conditions apply.
